Participant Waiver

By signing this document, I

• confirm that I have registered my child, or an individual for whom I have authorised decision making responsibility, for the Active Lifestyle event through Returned & Services League of Australia (Queensland Branch) (RSL Queensland); 
• authorise RSL Queensland to share my child's, or an individual for whom I have authorised decision making responsibility's, personal details with the event manager for the purposes of booking and registration; 
• understand that the event manager is responsible for the administration and delivery of the Active Lifestyle event and RSL Queensland are in no way responsible for participant health and safety or for the administration and delivery of the Active Lifestyle event; 
• confirm that my child, or the individual for whom I have authorised decision making responsibility, choose to participate in the program activities; 
• acknowledge that my child, or an individual for whom I have authorised decision making responsibility, is undertaking the Active Lifestyle event solely at their own risk; 
• release RSL Queensland from any claims or demands arising out of or in connection with my child's, or an individual for whom I have authorised decision making responsibility's, participation in the Active Lifestyle event as well as any damage, loss, or expense howsoever caused which may arise as a result of any participation in the Active Lifestyle event; 
• understand that the difficulty of this program is categorised as Green and I am confident in my child's, or an individual for whom I have authorised decision making responsibility's, ability to partake in this level of physical activity (please see registration form for further information); 
• confirm that my child, or an individual for whom I have authorised decision making responsibility, will abide by the rules and requirements of participation outlined by the event manager; 
• acknowledge that my child, or an individual for whom I have authorised decision making responsibility will be unable to participate in the Active Lifestyle event if they do not comply with the rules and requirements of participation outlined by the event manager and RSL Queensland will not be responsible for any loss or damage suffered as a result of my child, or an individual for whom I have authorised decision making responsibility, being unable to participate; and 
• consent to give and receive this document, including signatures to execute this document, by electronic communication in accordance with the Electronic Transactions (Queensland) Act 2001 (QLD). 

Talent Release Details

RSL Queensland may take photographs, video or audio recordings during the Active Lifestyles event. Please advise us whether you agree for your child or an individual for whom I have authorised decision making responsibility to be photographed or be in a video recording taken at the Active Lifestyles event. • This activity is open to Veterans, current serving Defence members, their partners and Children over 8+ years old.
• Children must be supervised by an adult at all times.
